  • Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
  • Photocopy of one (1) Government issued ID with signature.

AUTHORIZED REPRESENTATIVE (IF OWNER IS NOT AVAILABLE)

  • Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
  • Authorization Letter of the owner.
  • Photocopy of one (1) Government issued ID of registered owner with three (3) specimen signatures.
  • Government issued ID of the authorized representative with signature.

CORPORATE ACCOUNT

  • Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
  • Authorization from the company’s authorized signatory.
  • Photocopy of one (1) Government issued ID with three (3) specimen signatures of authorized signatory.
  • Company ID or Government issued ID of authorized representative.
